Condition & Prognosis Reports

The assessment and provision of a Physiotherapy condition, prognosis, and needs report is a serious and responsible matter. Our instructions are evenly split into Claimant / Defence. The responsibility remains to the Court to provide a reasonable and defendable package of care, whilst ensuring the Claimant is suitably cared for in the future.

These reports often require careful interaction with orthopaedic, care, accommodation, and other experts. Close and open communication between experts ensures all the claim reports present a sensible, reasoned, and joined-up set of needs to the Court.

It is important to listen to the patient and take time in your assessment.

  • Condition and Prognosis - cervical spine injury delayed diagnosis

    Cervical disc prolapse delayed diagnosis in A&E resulting in musculoskeletal and neurological impairment. Assessed the Claimant at home and provided a detailed condition, prognosis and Physiotherapy needs report. Met and discussed the needs with Claimant, legal team, and other experts to produce a joined-up claim for service.

  • Condition and Prognosis - Cauda Equina

    Neurological impairment from cauda equina following a negligent epidural procedure. Assessed the Claimant at home with husband and carers and provided a detailed condition, prognosis and Physiotherapy needs report. Met and discussed the needs with Claimant, legal team, and other experts to produce a joined-up claim for service after a Joint Meeting with Defence Physiotherapy Expert.

  • Condition and Prognosis - amputee needs

    Needs assessment for an amputee patient following an industrial accident. Assessed the Claimant at home with his daughter and provided a detailed condition, prognosis and Physiotherapy needs report. Conference meeting with Claimant, legal team, and other experts to produce a joined-up report for service to the Defence. Case settled.

  • Condition and Prognosis / Needs assessment - abdominal surgery rehabilitation needs

    Post-failed abdominal surgery and infection leading to poor trunk control and chronic pain. Assessed the Claimant at home with his mother / carer and provided a detailed condition, prognosis and Physiotherapy needs report. Case settled.

  • Condition and Prognosis - RTA ankle injury management

    Needs assessment of chronic pain and dysfunction in an ankle following an RTA. Assessed the Claimant at home and provided a detailed condition, prognosis and Physiotherapy needs report.

  • Condition and Prognosis / Needs assessment - amputee

    Above knee amputee following a work injury. Assessed the Claimant at home and provided a detailed condition, prognosis and Physiotherapy needs report. A joint report was also provided following a meeting with the Claimant’s Physiotherapist. Case settled.

  • Condition and Prognosis / Needs assessment - Toe surgery

    I assessed the Claimant in their home for their needs for the future rehabilitation of a big toe issue following negligent surgery. We met with the orthopaedic experts and legal team following the submission of a detailed condition, prognosis and Physiotherapy needs report.

  • Condition and Prognosis report following injuries suffered from falls in hospital

    The Claimant was not properly supervised and fell twice resulting in two hip fractures and subsequent care issues. Report on the condition, prognosis, and future Physiotherapy needs.
